There are two ways to reorganise the Apps on your iPhone.
On the iPhone itself you can press and hold an app icon and you will see that all your apps begin to wobble and those apps that you have added will have a cross appear on their icons. You can then drag and move icons within a page and from page to page. You can also delete apps by pressing the cross on the app icon.
This is pretty straightforward if you have a manageable number of apps. Those that have many pages of apps on their iPhone will find that this becomes a bit unwieldy. An alternative is to use iTunes. When the iPhone is connected to your Mac or PC through iTunes you can highlight the iPhone in the Devices list on the left hand side and hit the Applications tab on the main page. This will bring up a graphical overview of all your pages of apps, exactly as you see them on your iPhone. You can then rearrange or delete apps whilst viewing multiple pages at a time.
When you're done just remember to click the Sync button to ensure your changes are saved.
